The Online Patient Portal allows you to access your medical record and communicate with our practice securely. You can now schedule a Video Visit with one of our providers by calling our office. 

Once logged in you will be able to:

• Update Contact information

• View upcoming and past appointment

• View your visit details and any completed test results

• Ask a question

• Confirm or edit your demographics and personal history information

We would love our patient's to use their patient portal.  This is a great tool to view your medical record with our practice and print them to share with another medical provider. 

 

In order to gain access to your portal, please request it by calling our office, requesting our office call you through the contact form here, or ask us at your next appointment.  Our staff will send you an e-mail that will grant you access to your portal. 

 

This e-mail link is only valid for 72 hours, so if you miss the window to sign in to your portal, just contact us and we will resend.
